Pentavite DIRECTIONS FOR USE Administer dose via syringe provided directly into child's mouth or add to a teaspoon of milk or fruit juice. Rinse syringe when finished and allow to dry. 1 to 3 years: 3.5 mL daily or as directed by your healthcare professional. 4 to 12 years: 5 mL daily or as directed by your healthcare professional. Do not shake. Store upright below 25°C protected from heat and light. Refrigerate after opening. Do not use if seal around cap is broken. Use within 9 weeks of opening. Each dose contains: 3.5 mL 5 mL (1-3 yrs) (4-12 yrs) Thiamine hydrochloride 1.8 mg 2.5 mg equiv. to thiamine (Vitamin B1) 1.56 mg 2.23 mg Riboflavin sodium phosphate 1.75 mg 2.50 mg equiv. to riboflavin (Vitamin B2) 1.3 mg 1.8 mg Nicotinamide (Vitamin B3) 10.5 mg 15 mg Pyridoxine hydrochloride 525 microgram 750 microgram equiv. to pyridoxine 432 microgram 617 microgram (Vitamin B6) Ascorbic acid (Vitamin C) 42 mg 60 mg Colecalciferol (Vitamin D3) 3.5 microgram 5 microgram Ferric pyrophosphate equiv. to iron 2.5 mg 3.6 mg Contains saccharin sodium, sorbates, sulfites and 2.44 g of sugar alcohols (sorbitol and mannitol) per 5 mL dose.
